8.) Building and room related to the request Room
* 9.) Short Description (up to 50 characters only)

Part B (required for requests that pass the test ...)

This part of the form must be completed for ANY project that involves a change of use, change to structure, installation of equipment, alteration to or attachment of anything to the building fabric, or connection to building services.

12.) Is there a change in the number of people in the area? Is this an increase or decrease?
13.) Is there any change proposed to the fabric or structure of the building?
14.) Is equipment to be installed? If so what does it weigh? What services connections does it require?
15.) What are the services requirements for all the work - mechanical, electrical, communications, hydraulics?
16.) Are there any specialist requirements or hazards related to the works?
17.) Does this work involve relocation of personnel?
